Academic Year Statement: Military
Any student required to submit an “Academic Year Statement” (as part of the Health Professionals Scholarship Program, HPSP) should provide and request completion of this form through the Office of the Registrar. These requests will be processed in the order in which they are received (by the Office of the Registrar).
