Comparison of Immunological Profiles Between Commercial and Standard / Historical Lines of Turkeys: The potential application of this study involves the use of genetic markers to help characterize immune function of individuals. This three-year project was awarded by the American Livestock Breeds Conservancy and runs to May 2005. The Principal Investigator is Dr. Robert Gogal. This project runs through both VCOM and Virginia Tech.


Ecstasy's Neurotoxicity: Role of Hyperthermia:
The long-term goal of this project is to clarify the role of hyperthermia in the acute and chronic toxicities associated with MDMA exposure. This research is significant, because understanding the mechanisms and consequences behind MDMA-mediated hyperthermia will assist in identifying several new targets for the therapeutic treatment of MDMA induced toxicities. The Principal Investigator on this project is Dr. Jon Sprague, and it is sponsored by the National Institute on Drug Abuse, NIH. This grant will run to March 31, 2007.

Dr. Christopher Reilly

Histone Deacetylase Inhibitors are the Therapeutic Targets in SLE:
Investigators are testing the hypothesis that an imbalance of histone acetyltransferase (HAT) and histone deacetylases (HDAC) activity may contribute to pathogenesis of systematic lupus erythematosus (SLE). This project is a subcontract to Wake Forest University Health Sciences and is sponsored by the Alliance for Lupus Research. Dr. Christopher Reilly is the VT/VCOM Principal Investigator. This project began will run through July 2005.

Histone Deacetylase Inhibitors Are The Therapeutic Targets In SLE:
This grant, a competitive renewal application to the above project, was recently awarded by the NIH as a separate project, and will be active for one year. Wake Forest University is the prime, and Dr. Christopher Reilly is the Virginia Tech/VCOM Principal Investigator.

Immune Protection against MNU-Induced Digital Defects:
The goal of this project is to clarify gene and cytokine-related mechanisms by which the maternal immune system has effectiveness for intervention and/or prevention of chemical-induced birth defects in rodents. This grant is funded through the NIH's Division of Comparative Medicine, National Center for Research Resources, and Dr. Renee Prater is the Principal Investigator (a mentor and co-mentor on the project are faculty at the College of Veterinary Medicine). This five-year project runs until March 31, 2007 and is a joint project between VCOM and Virginia Tech.

Inhibition of IRF-1 in MRL/Lpr Mice:
Dr. Christopher Reilly
is the Principal Investigator on this project, and Drs. Arben Santo and Robert Gogal are assisting Dr. Reilly as Co-Investigators. The goal of this research is to inhibit the inflammatory processes associated with lupus nephritis, thus reducing and possibly alleviating disease progression. This three-year grant was awarded by the National Arthritis Foundation (with an additional two-year renewal possible). This grant is a joint project between VCOM and Virginia Tech.

Inhibition of Lupus Nephritis in IRF-1 Deficient Mice:
This project aims to clarify the role of interferon-gamma regulation on lupus nephritis. Dr. Christopher Reilly is the Principal Investigator on this project, funded by the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIH). The grant is a joint project between VCOM and Virginia Tech and runs to May 2005.

Macromolecular Interfaces with Life Sciences (MILES): Oxidative Processes:
MILES is an Integrative Graduate Research and Education Traineeship (IGERT) sponsored by the NSF. The purpose of this program was to equip a new generation of professionals with skills that will move society forward into an era of sustained health, well-being, and quality of life through technological advancements. The program equipped future leaders in science and engineering with the broad-based skills necessary to transcend conventional boundaries. Dr. Hara Misra was a Co-Investigator on this one-year project, which ran through Virginia Tech (Principal Investigator: Susan Duncan). This project was completed in February 2005.

Microglial Activation in Traumatic Brain Injury:
Dr. Beverley Rzigalinski is the Principal Investigator on this project, which is sponsored by the National Institute of Neurological Disorders and Stroke, NIH. This grant examines the signal transduction pathways responsible for traumatic brain injury-induced microglial activation and determines the consequences of activation on neuronal injury. This research could permit the development of more effective strategies aimed at reduction of secondary injury after traumatic brain injury by shifting the balance of microglial effects from neurotoxic to neuroprotective. This is a joint project between VCOM and Virginia Tech and will run through June 2006.

Dr. James Mahaney

Molecular Mechanism of Calcium Transport Regulation in the Heart: Dr. James Mahaney was the Principal Investigator on this project, which was sponsored by the American Heart Association. The goal of his research program was to help clarify the molecular mechanism of calcium transport regulation in the heart. This work involved the Ca-ATPase and phospholamban; a number of heart-disease states are linked directly to abnormal phospholamban Ca-ATPase interactions. This grant was a joint project between VCOM and Virginia Tech, and ended on December 31, 2004.

Nanoparticles As Antioxidants And Promoters Of Cell Longevity:
Dr. Beverley Rzigalinski is the Principal Investigator on this project, which is sponsored by the National Institute on Aging, NIH. In this proposal, nanoscale materials science and engineering have merged with cell biology to intervene in a common biomedical pathology, that being aging and free radical-induced cell dysfunction. The aims within this project will provide substantial insight into the pathology of aging and age-associated disorders and initiate a nanotechnological approach to pharmacotherapy. This VT/VCOM joint project will run through July 2007.

Prevention of Neural Tube Defects by Immune Stimulation:
Dr. Theresa Hrubec
is the Principal Investigator of this project, which is sponsored by the National Center for Research Resources, NIH. The overall goal of the study is to determine how environmental toxicants change the cellular events during gestation and lead to developmental birth defects. This research aims to determine the cellular and molecular changes responsible for the defects and how maternal immune stimulation normalizes the cellular events to prevent developmental abnormalities. This five-year grant is a joint project between Virginia Tech and VCOM, and will run through July 2008.
Regulation of Calcium Transport in Cardiac Muscle:
In this project, the cDNA coding of the Ca-ATPase was modified so that the reactive cysteines were replaced with serines, permitting the site-specific incorporation of precise cysteines that can be used for the covalent attachment of dyes. This research was awarded by the NIH's National Institute on Aging to Pacific Northwest National Laboratory (PNNL). Dr. Mahaney was the subcontractor Principal Investigator. This subcontract was a joint project between VCOM and Virginia Tech, and ended on April 30, 2005.

Signal Processing Methods for Complex Phenotypes:
This project was submitted by Virginia Tech to the National Institute for Biomedical Imaging and Bioengineering, NIH. Dr. Gogal is an Investigator on this proposed research (Principal Investigator: K. Duca). This two-year project was recently awarded, and will begin on June 1, 2005.

Three-Nitrotyrosine in Aging of Skeletal Muscle and Heart:
This research was awarded by the National Institute on Aging (NIH) to Washington State University. Virginia Tech is the subcontracting institution and Dr. Mahaney was the Principal Investigator. This two-year project ended in March 2005, and ran through both VCOM and the Department of Chemistry at Virginia Tech.

Biomedical Sciences Division: Pending Research Projects

Dr. James Palmieri

F. tularensis gene expression and transmission after phagocytosis by Acanthamoeba:
Francisella tularensis, one of the most pathogenic organisms known and the causative agent of tularemia in man, is classified by the CDC as a Category A biological weapons agent. This significance of this study is to understand the relationships between acanthamoebae, as a vector for F. tularensis, and its ability to serve as a means for biological transmission of tularemia through drinking water. This will shed light on how F. tularensis survives and is transmitted in nature and on its potential as a biological weapon to exposed populations through drinking water. Dr. James Palmieri is the principle Investigator on this project and Dr. Tom Inzana, CMMID, Virginia Tech Vet. School, is co-principle investigator. If awarded, this two-year project will be run through both VCOM and Virginia Tech.

Identification Of The Determinants Of Microaerophilic Growth In Human Gastrointestinal Pathogens:
Dr. Richard Seyler submitted this grant to the Goldman Philanthropic Partnership's Charles E. Culpeper Biomedical pilot Initiative. This study proposes to modify and use a transposon-based mutagenesis system to identify genes responsible for the low-oxygen requirement of two gastric pathogens. Identification of the genes involved in microaerophilic growth is an important first step toward the development of new, more specific therapies for the diseases caused by these bacteria. This is a joint project between VCOM and Virginia Tech, and will run for 1 year if awarded.

Prenatal TCDD and Postnatal Autoimmune Disease:
Dr. Prater and Dr. Gogal are Investigators on a grant submitted by Dr. Steven Holladay at Virginia Tech's College of Veterinary Medicine. This grant was submitted in August 2004 to the NIH. It will run for three years if awarded.

Regulation Of The Cardiac Calcium Pump By Phospholamban:
This project was submitted by the University of Maryland, School of Medicine to the NIH. Dr. James Mahaney is the subcontractor Principal Investigator for Virginia Tech. This grant was submitted in June 2004 and, if awarded, will run for five years.

Clinical Sciences Division: Current Research & Outreach Projects

Access to Electronic Health Information:
The goal of this project was to increase access to electronic health information and improve health literacy for an underserved region of Appalachia. A partnership, formed between VCOM, the Montgomery-Floyd Regional Libraries, and the Blacksburg Electronic Village, facilitated training in the access of health information via computer for consumers and healthcare providers within Montgomery and Floyd Counties, as well as Southwestern Virginia as a whole. This project was a subcontract under the University of Maryland Health Sciences and Human Services Library's National Network of Libraries of Medicine sponsored by the National Library of Medicine, NIH. Dr. Alfred Pheley is the VCOM Principal Investigator. This project ran for two years and was completed in February 2005.

Addiction Severity Index in Parolees and Probationers, District 28, Radford, Virginia:
The goal of this project is to complete a census of the Addiction Severity Indexes and comment sections collected from District 28 after 2001. This data will be compared to the Addiction Severity Indexes from the Principal Investigator's completed OxyContin® Use, Abuse, and Diversion in Southwest Virginia research study. It is hypothesized that a group of probationers and parolees with drug and alcohol abuse problems will not differ significantly in their scores on the Addiction Severity Index from a group of prisoners incarcerated for OxyContin® possession or diversion. In addition, it is hypothesized that individuals from District 28 who are abusing prescription opioids and benzodiazepines will have a history of drug and alcohol abuse prior to their abuse of prescription drugs. This one-year research project was recently awarded by Purdue Pharma, and Dr. Martha Wunsch is the Principal Investigator.

Appalachian Medical Digital Library Internet Access:
The primary objective of this project is to provide VCOM with equipment, staff, and support to extend current resources to eight community hospitals located throughout underserved regions of Appalachia. This project will create an internet based Digital Library Network (DLN) to enhance library resources and medical information for the region. This 1.5-year project is funded by the NIH's National Library of Medicine, and will be completed in October 2005. Dr. Alfred Pheley is the Principal Investigator of this project.

Redesign and Continuing Education on the Substance Abuse Toolbox:
A Memorandum of Agreement was signed between the Virginia Primary Care Association (VPCA) and VCOM. Dr. Martha Wunsch will redesign the current Substance Abuse Toolbox, a program designed to help health practitioners identify and manage patients in their practices with substance abuse problems. The redesign is important to enhancing the utility of the Toolbox to physicians. The MOA provides funding to VCOM to support Dr. Martha Wunsch's redesign efforts and time associated with four training session conducted in Southwest Virginia.

Smyth County, USA: A Model for Partnering in Health: This project is a community-based initiative focused on developing new healthcare resources through collaboration between Smyth County and VCOM and expanding existing services by optimizing linkages between healthcare resources that already exist in the County. The specific one-year goals of the project are: (1) to increase access to healthcare for all needy residents of Smyth County, (2) to expand treatment of chronic disease and disability, (3) to enhance resources for stimulating disease prevention and health promotion, (4) to expand assessment of and education about environmental risk factors, and (5) to ensure the long-term sustainability and growth of this collaboration. The Principal Investigator on this project is Dr. Paula Scariati. Award was made in December 2004, and the one-year project will run to January 2006.

Biomedical Basis of Mild Traumatic Brain Injury:
This grant was submitted by Simbex Inc. to the Bioengineering Research Partnerships program at NIH. VCOM is a subcontractor to Simbex and the VCOM Principal Investigator is Dr. P. Gunnar Brolinson. Dr. Pheley is an Investigator on the grant. This proposal was submitted in January 2005, and will run for five years if awarded.

Gender Differences in Musculoskeletal Stability:
This grant was submitted by Virginia Tech to the NIH. VCOM is a subcontractor on proposed project, and Dr. Brolinson is the VCOM Principal Investigator and Dr. Francine Anderson is an Investigator. This grant was submitted in May 2004 and, if awarded, will run for five years.

Health Care Opportunities in Virginia's Appalachian Region:
This grant was submitted by VCOM to the Health Resources and Services Administration (HRSA) on February 22, 2005. Dr. Pheley is the Principal Investigator and Dr. Gogal is the Co-Investigator. If awarded, this project will run for three years.

Opioid Mortality in Southwestern Virginia:
The long-term goal of this research is to characterize a population of deaths in rural southwest Virginia that are the result of misuse and abuse of prescription opioid medications and characterize their lethality in combination with other psychoactive substances. Dr. Martha Wunsch submitted this revision to the National Institute on Drug Abuse, NIH in November 2004. If awarded, this project will run for two years.

Project REMOTE (Rural Enhanced Model for Opioid Treatment Expansion):
This grant was submitted by the Virginia Dept of Mental Health, Mental Retardation, & Substance Abuse Services to the Substance Abuse and Mental Health Services Administration (SAMHSA). Dr. Wunsch is the VCOM Principal Investigator for this project. If awarded, this will be a three-year project.

Teaching Med Students Prevention - Right From the Start:
Dr. Paula Scariati submitted this proposal to the Association of Teachers of Preventive Medicine / CDC in April 2005. If awarded, this three-year project will focus on creating an empirically-validated, gold standard prevention curriculum for medical students. This will be achieved by developing and teaching the foundational tools required to understand Preventive Medicine; developing and teaching methods for applying these tools to master the discipline of disease prevention and health promotion; and modeling, promoting, and supporting a healthy lifestyle. If awarded, Virginia Tech will be a subcontractor to VCOM on this grant.

Telemedicine: Utilization and Barriers in Rural Virginia:
Dr. Rebecca Davis and Dr. Pheley submitted this application to the Virginia State Office of Rural Health in August 2004. This project will run for five months, if awarded, and the requested direct funding is $4,790. This project also has a cost-share portion: VCOM will pay a total of $3,240 to cover the salaries of both Principal Investigators.

The Health Matrix - Engaging Diverse Partners in Community Well Being:
This project will be focused on testing the effectiveness of a community collaborative model to improve health indicators in a rural Appalachian county, Smyth County, Virginia. This project will develop a partnership between a group of community leaders, residents and an academic institution to create a coalition that acts as a single guiding force in the community. Dr. Paula Scariati submitted this proposal to the NIH in May 2005, and if awarded, it will run for two years.

VCOM Community Health Project, Lare Njoro, Kenya:
Dr. Joseph Cacioppo submitted this grant to the Mercy Center Foundation in June 2004. Dr. James Palmieri is an Investigator on this project. If awarded, this project will run for three years.

Current Outreach Projects

A Rural Appalachian Model Medical Education Program:
This grant is helping to establish a rural Appalachian model medical education program and create an academic health center environment in four community hospitals in Southwest Virginia. The program will include medical student training and continuing medical education and physician development through the Buchanan General Hospital, Russell County Medical Center, Norton Community Hospital, and Lee Regional Medical Center. This one-year project was awarded to VCOM by the Commonwealth of Virginia Appalachian Regional Commission in April 2004, and will run to August 2005.

Rural Clinical Training Program:
This grant has recently been awarded via federal appropriation to VCOM and Dr. Tooke-Rawlins (through HRSA). This program will establish new medical training sites in rural areas of Southwestern Virginia as well as West Virginia. This funding is for one year. This project is expected to being in October 2005.

Other Significant Awards

Robert Wood Johnson Health Policy Fellowship:
This fellowship was awarded to Dr. Pheley in February 2005. Dr. Pheley will participate in health policy processes at the federal level. The experience from this fellowship will provide leadership to improve health, health care, and health policy at all levels. The one-year fellowship begins September 1, 2005. An additional two years may also be awarded for leadership development activities.

Library Training for Hospital Staff:
Ms. Elaine Powers, VCOM Librarian, was recently awarded a training grant from the National Libraries of Medicine. This was awarded in February 2005 for a training session held at VCOM on March 17, 2005. Hospital administrators and physicians from all VCOM-affiliated hospitals were invited to this training session on electronic library databases.

State or Regional Exhibiting Award:
Ms. Elaine Powers was recently awarded this grant to secure exhibit space at the May 2005 VOMA Conference. This award is sponsored by the National Libraries of Medicine Southeastern/Atlantic as part of its outreach program. This program allows network members to exhibit at state or regional meetings of health professionals, library associations, or health consumers.

Internally-Funded Research Projects

Altered Peripheral Cytokine Profiles Associated with Heat Stress in Athletes: Identification of a Circulating Marker for Exercise Induced Endotoxemia:
This project has recently been recommnded for funding by the VCOM Interdisciplinary Review Committee (IRC). Drs. Renee Prater and Gunnar Brolinson are the Principal Investigators. This project aims to define the association between ambient heat stress index, thermal microenvironment of helmet/padding of football equipment, and core body temperature and development of adverse gastrointestinal signs of exercise-induced endotoxemia.

Body Composition Assessment in High School Wrestlers: Relationships of Anthropometrics and Body Hydration:
Drs. Del Bolin and Alfred Pheley (Jesse Donneworth, M.S., and Amy Freel from VT Athletics) received funding for this grant by the VCOM IRC in March 2005. This current research project proposes to validate bio-impedance among euhydrated high school wrestlers.

Brain Injury Evaluation in Real Time of Sports Trauma:
This project is internally-sponsored by both VCOM and Virginia Tech. Dr. Gunnar Brolinson is the Principal Investigator of this project, which is currently in its second year. VCOM allocated Dr. Brolinson funds for purchasing the HITS system. A publication is under review, and the Principal Investigator expects to apply for extramural funding in the near future, based on the data from this project.

Caring for Special People Webpage:
Dr. Ava Stanczak began this VCOM-sponsored project in November 2003. This three-year project will run through December 2006. In this project, Dr. Stanczak has been developing a website for use by parents, caregivers and health care providers that will eventually showcase approximately 900 childhood orphan diseases and their care. One of the goals of the new website is to have medical evidence based information and best practices information all on one site with easy access.

Experimental Infection of Acanthamoeba Castellanii and A. astronixis with Virulent and Avirulent Strains of Francisella tularensis:
In March 2004, the VCOM IRC awarded funding to Dr. James Palmieri for for this project, with the plans that an application for extramural funding will be forthcoming. Francisella tularensis is the organism that causes cutaneous and pulmonary tularemia in humans. This project will determine the mechanisms on how F. tularensis survives and colonizes and how it maintains itself in its silent cycle in nature.

Measuring Ankle Stability to Predict Ankle Sprains:
Dr. Brolinson began this VCOM-sponsored project in June 2003, and it is currently in its second year. In this project, a device is under development that can quantify ankle stiffness and ankle reflex behavior. This device is being used to measure the ankle stiffness and ankle reflex behavior of members of the Virginia Tech athletics teams during the annual preseason physicals.
